
The 18th Annual Limbs for Life Golf Tournament was scheduled for October 19th. After speaking with most of you and gathering your input, we have made the difficult decision to not hold the tournament this year. The impact of the current environment on you - our sponsors, partners and friends - is significant.
We utilized a survey model to capture specific data in making the decision. Although several would attempt to participate if we held the tournament, most would need to reduce the level of sponsorship. With the feedback provided, the committee determined it is not economically feasible to hold the Limbs for Life golf tournament this year.
We value our relationship with you and want to do what’s in the best interest of Limbs for Life and each one of you. We are so grateful for your support and participation through the years and we will return. In fact, we have reserved October 18, 2021 at Gaillardia for our next tournament date. Mark your calendar, plan for your participation, and join us then. We will look forward to it!

AMPUTEES IN NEED
We provide financial assistance for prosthetic care provided by local professional partners to qualified, approved applicants at no cost to the amputee.
Primary eligibility requirements:

1. Am I eligible for assistance through Limbs for Life?
Limbs for Life (LFL) provides assistance for lower limb amputees. Applicant must be a U.S. citizen or a permanent legal resident of the U.S. Applicant must have no other means to pay for prosthetic care including Medicare, insurance coverage or state assistance. Applicant must work with a prosthetist or clinic that agrees to accept LFL payment as full payment for their services.
2. How do I locate a prosthetic clinic that will accept LFL payment?
Ask your selected provider to review the application and determine if they will agree to work within the LFL requirements. Your clinic may call LFL for more information about our application process. LFL maintains a list of clinics that have worked with our clients in the past. View the list of clinics or call 1-888-235-5462 for referrals in your area.
3. How do I apply for assistance?
Complete the application and have your prosthetist or clinic complete their section. Include a readable copy of your photo ID; If not a US citizen, you must send a copy of your Permanent Resident Card. Send application, proof of household income (award letters, benefit statement, pay stubs) and photo ID to Limbs for Life Foundation by mail, email or fax.
4. How long will it take to get my new leg?
Our minimum waiting period is 90 – 120 days following receipt of your complete application. Applicants with felonies will be reviewed by an advisory committee. Please contact our office to confirm receipt of your application. You will be contacted to verify your information closer to your processing date. This will take several months.
5. What if I move or change my prosthetist before I receive my limb?
Contact the LFL office with your updated information. Your new prosthetist will need to complete the prosthetist portion of the application form; a new confirmation letter will be issued to the new prosthetist.
6. What if I gain other coverage for my prosthetic care prior to receiving confirmation from LFL?
Notify the LFL office as soon as possible so that the funds can be used to help another amputee.

THE VITAL ROLE OF PROSTHETIC CLINIC PARTNERSHIPS
Through partnerships with prosthetic clinics across the country, Limbs for Life provided payments of $608,500 to help over 2,788 clients get back on their feet in 2019 alone. Collaborating clinics in 28 states provided prosthetic limbs, fittings, adjustments and additional services to clients approved for services from our foundation. Clinics donate their time and talents to help the amputees.
Limbs for Life Foundation shares components and parts as available and provides a financial bridge between low-income amputees and the clinics providing the quality prosthetic care needed. With appropriate care and assistance, amputees have the opportunity for a productive and independent lifestyle.

How can we serve you and your patients?
Limbs for Life is a resource to assist prosthetic clinics to provide care for uninsured and under-insured amputees. We can provide used component parts, at no cost, to prosthetic clinics and practitioners willing to fabricate a limb for an amputee in need. We also partner with clinicians who travel abroad to do mission work internationally.
Limbs for Life provides direct financial payment to clinics for qualified applicants at a fixed contract amount of:
$2,500 for a below-knee prosthesis $3,500 for above-knee prosthesis
Specific terms and agreement for the clinic partnership are included in the Client Application.
Limbs for Life also provides donated components, as available, to lower your clinic’s expenses. Clinicians may request used parts for repair or replacement on existing limbs, even when a new limb is not required. We ask our clinic teams to limit requests to those who cannot otherwise afford needed parts.

Meeting a world of amputee needs

An important part of our program at Limbs for Life is providing used prosthetic components to developing countries. Each week Limbs for Life receives shipments of used limbs that are no longer needed by the original client. The limbs are dismantled and broken down into component parts and made available for worldwide humanitarian efforts led by foundations, groups and individuals. While laws regulate reuse of certain components in the United States, many items may be utilized in foreign countries. Our partnerships with prosthetic clinics and several non-profit agencies around the globe allow Limbs for Life, based in Oklahoma, to impact the lives of amputees on five continents.
